Failing to learn from failure

Warnings from the 9/11 Commission members that the US is at great risk of more attacks raises serious questions about whether the Government has learned from mistakes. But it's also a reminder that most organizations have trouble analyzing and dealing with failure. You don't just see it in politics but in commercial life too. It's why fraud, malfeasance and corporate meltdown will always be with us. No legislationlinks can stop that. And just remember, no-one has yet invented a new way of going broke.
